UNDERSTANDING YOUR OPTIONS

What is community support?

Getting out and staying connected can require a little extra planning and support. Made4Care provides community support in Peterborough and Cambridge, helping adults take part in agreed everyday activities and maintain their links with the people and places that matter to them.

You do not need to have every detail worked out before you call. We can talk through the parts of daily life that are becoming difficult, what is already working well and what kind of assistance would feel right.

PRACTICAL HELP, PERSONAL ATTENTION

How community support can support you

Your assessment helps us agree which tasks to include and how much time is needed. Support is shaped around your abilities and preferences.

Local outings

Accompaniment to places you choose, subject to assessment and an agreed plan.

Shopping trips

Support to take part in shopping rather than leaving all the decisions to someone else.

Appointments

Company and agreed practical assistance when attending an appointment.

Activities & interests

Discuss the hobbies, groups or places you would like to remain connected with.

Travel planning

Agree transport, accessibility and timing in advance. Transport is not automatically included.

Building a wider routine

Community support may be combined with companionship at home or other agreed care.

THE MADE4CARE APPROACH

A good outing starts before you leave home

We discuss the destination, journey, access and assistance you may need. That includes mobility equipment, timings and any costs or transport arrangements. Planning together helps everyone understand what is involved before an outing is agreed.

  • We listen to your preferences before agreeing support.
  • Your care plan records the help and routines that matter.
  • With consent, family members can be involved.
  • Contact the team when your needs or circumstances change.
